One record per preparer: full_name, business_name (the DBA), credential (Certified Public Accountant, Enrolled Agent, Attorney, Supervised Return Preparer, Certifying Acceptance Agent, Enrolled Retirement Plan Agent — or "PTIN holder" when the IRS lists none), city / state / zip_code / country, street, phone, website, the raw IRS credential codes, and the Annual Filing Season Program flag.
The IRS publishes the roster of preparers holding a current PTIN but never the PTIN number itself, so license_number is empty by design and every record is status: active with status_basis saying exactly that.
